After completing the application form, return it in person or mail it to the University together with the following supporting documents:<\/p><ol><li>A copy of the child\u2019s birth paper, a photograph and five return envelopes. (Please write the child\u2019s name, address and $2 stamp on each envelope on the envelope cover)<\/li><li>Application Fee: HK$40 for 123<\/li><\/ol><ul><li>Payment at the school: Please pay the fee of $40 in cash when you submit the form at the university.<\/li><li>By post: It can be paid by cheque, please write \u201cEVI Services Limited\u201d on the letterhead (please indicate the student\u2019s name and grade on the back) and return it to the university together with supporting documents.
