Please visit our school in person to request the “Application Form” by fax/ email or on this website. After completing the application form, return it in person or mail it to the University together with the following supporting documents:

1. A copy of the child’s birth paper, a photograph and five return envelopes. (Please write the child’s name, address and $2 stamp on each envelope on the envelope cover)
2. Application Fee: HK$40 for 123

• Payment at the school: Please pay the fee of $40 in cash when you submit the form at the university.
• By post: It can be paid by cheque, please write “EVI Services Limited” on the letterhead (please indicate the student’s name and grade on the back) and return it to the university together with supporting documents. (Please post with a $6 stamp)
